The following two documents are the RSTC medical form which must be completed by anyone desiring to partake of scuba diving.  The first document lists various conditions which may prohibit a person from participation in scuba diving activities.  If any of these conditions are answered "yes" the participant must have the second document signed by a physician prior to any in water activity.
